The REACH Clinic Reverse Site Visit in February of 2025 led a group of similarly focused clinics to form the Culture of Care Collaborative. The Culture of Care Collaborative is a group of clinics from cities across the country that facilitate access to care post hospital discharge for individuals who have been injured through violence. The model is one of co-located services to address and facilitate access to biopsychosocial healing from violence for individuals, also working with their families.

Tuesday, April 28: Qualitative Research Training Introduction and Overview (10am-3pm)

This session is intended to introduce individuals and teams to frontline-led qualitative research in the realm of violence intervention and prevention. The Denver Youth Program (DYP) team has trained ten of our frontline Violence Prevention Professionals (VPPs) and Violence Prevention Specialists (VPSs) and two professional collaborators over the course of the past two years. This session will:

  • describe our methods and rationale for forming a frontline-led research team
  • share our publications and current projects to provide an overview of our research agenda
  • define the key attributes for success in establishing our research team
  • assess readiness for building a research team at your site
  • detail an incremental readiness plan for scaled research at your site
  • discuss opportunities for shared research as a complement to or building block toward independent site-specific research

Wednesday, April 29: Shared Strategies in Clinical Models of Care for Victims of Violence (9a-4p)

This session is intended to be a working group to bring together the Culture of Care Collaborative partners and sites who serve clients and families through multidisciplinary models of care to support holistic healing post hospital discharge to discuss success and challenges, ideas and logistics, and shared strategies and initiatives. Participants need not be participating in this model of care currently, though discussion is intended to be held at this depth and nuance of experience.

Thursday, April 30: Community Presentations and Conversation (10a-3p)

This day will consist of presentations by partner models from around the country, including academic partners, healthcare collaborators, and local community members engaged in holistic recovery practices for individuals, families, and communities injured by community-based interpersonal violence. Events for this day will be held at the University of Colorado Anschutz Medical Campus.

Healing Together: Collaborative Healing Models for Individuals and Families Affected by Violence

Presentation Description:

There exist a variety of models and approaches to facilitate access to resources needed to support holistic healing for individuals who are injured through community-based interpersonal violence. Whether participants work through one-to-one case management, within a trauma recovery center, or in a community-based clinic, this session will facilitate discussion regarding key components of holistic recovery for individuals, families, and communities. This workshop will build upon insights from the Culture of Care Collaborative, which was born out of a reverse site visit hosted by the REACH Clinic in Denver, Colorado in February of 2025. The reverse site visit included representatives from five clinics across the U.S. with the purpose of exchanging models of care, identifying shared challenges, and co-creating solutions for long-term recovery beyond hospital discharge. This two-day convening attended by over 100 participants from 38+ organizations and resulted in the Culture of Care Collaborative. Participants will discuss replicable strategies for post-discharge care, particularly through the lens of co-location of wraparound services, community-academic partnerships, and multisector collaboration to strengthen transitions from inpatient to outpatient care. Attendees will also discuss how a culture of care can best be cultivated in their own work.

Learning Objectives:

1. Identify key components of holistic, community-based healing for survivors of interpersonal violence.

2. Analyze facilitators and barriers to cross-sector collaboration in post-hospital care settings.

3. Apply lessons from the Culture of Care Collaborative to develop or enhance wraparound service models in participants' own communities.

Mini-visit: Culture of Care Collaborative Meeting - Sponsored by the UNC Injury Prevention Research Center (Wednesday, April 15 8-9am)

The Culture of Care Collaborative is a group of clinics from cities across the country that facilitate access to care post hospital discharge for individuals who have been injured through violence. The model is one of co-located services to address and facilitate access to biopsychosocial healing from violence for individuals, also working with their families.

This meeting will convene a group of individuals affiliated with the Culture of Care Collaborative for conversation, updates and planning. It is also open for anyone interested in learning more about the model.
